Mammogram screening: Z12.31, encounter for screening mammogram

Mammogram DIAGNOSTIC: R92.8, abnormal mammogram

Colonoscopy screen & Cologaurd: Z12.11, colon cancer screening

FIT testing/ Immunochem fecal: Z12.11, Immunochem fecal occult screen

Hepatitis C screening: Z11.59, Need for Hep C screening

Lung cancer screening (CT lung screen): typically use Z72.0, Tobacco use

             For MEDICARE patients, Lung screens are only covered from ages 50-77

UDS (Urinary drug screen): Z79.899 High risk med use or Z79.897 Long term use of opiate analgesic

Vaccines

For commercial insurance

• Flu- Seasonal (October- March)

• Prevnar 20

• Hepatitis B

• Hepatitis A

• Shingrix

• MMR

• Covid-19

• TDAP

• TD

• Meningococcal A, C, Y, W

For any other vaccine the patient will need to go to a pharmacy to receive.

For Medicare

Patients who are on MEDICARE can NOT receive some immunizations at the office. If a patient is on Medicare, the only vaccines that we can give them are

• Influenza (Flu)

• Covid-19

• Pneumococcal

For ANY other vaccine Medicare patients MUST go to the pharmacy to receive. They do not need an order.

Point of Care Testing (testing available in the office)

• A1C

• Urinalysis

• Flu A+B

• Covid-19

• Pregnancy

• Strep A
